Payroll Deduction Forms

Here is a list of payroll forms.

• Payroll Deduction Authorization - General - Authorizes Golden 1 Credit Union to automatically withdraw money from your direct deposit to distribute to Golden 1 loans or accounts. Use this form to add/change/delete debit originations.
• U.C. Davis / Med Center - Payroll Deduction Authorization-  Authorizes the payroll department to deduct and transmit loan payments or deposits to Golden 1 Credit Union.
• U.C. San Francisco Payroll Deduction Authorization - Authorizes the payroll department to deduct and transmit loan payments or deposits to Golden 1 Credit Union.
