Update Beneficiaries

To add a beneficiary or change your current beneficiary, please complete the Beneficiary Designation Form.

Once completed, return it by mail or in person to any Golden 1 branch along with a copy of the identification of all account owners.

Note: The Beneficiary form must be signed by all account owners before returning to the Credit Union. When completing or receiving these updates in branch, all owners must be present with valid photo Identification.
